Procedure

  • After you have requested an appointment (see Appointment), we will schedule a short telephone call to help us decide if there is a fit and make a first assessment whether I will be able to offer you suitable treatment.

  • If so, we will make an appointment for an intake within the following few weeks.

  • With the information from the intake, I will fill out the HoNOS+ questionnaire (in line with the care performance model) to see which level of care is needed, and whether I can provide this. 

  • If I can provide suitable care and you would like to work with me, we will draw up a treatment plan during our next appointment (within 1-2 weeks), based on your goals and wishes. This will serve as a (flexible) guideline for the treatment.

  • Every now and then I will ask you to fill out a questionnaire (not mandatory, but useful) to give us insight into how you are doing and whether we are on the right track.

  • As a health care psychologist, I aim at short-term treatment consisting of 5-15 sessions. 

  • Should the information from the intake and the HoNOS+ questionnaire point out that different care is needed, I will assist you in finding suitable treatment.

Privacy

As a gz-psychologist I adhere to privacy regulations according to the GDPR and WGBO (the law concerning medical treatment agreement). This means that no information will provided to third parties without your permission, for example. However, with your permission, I will inform your referring doctor about the start and end of the treatment, and will consult with your doctor if this is useful. Any letters to your doctor will be discussed with you for your approval. You will also receive copies of these letters if you wish. You can choose to sign a privacy statement at the start, so that no information is passed on to the NZa and no information about type of care will be given to your insurer. I will let you know more about this before and during the intake. See this document for further information.

Complaints

If you are not satisfied with the treatment, please let me know. We can discuss possible adjustments so that you will be satisfied. If we cannot resolve the issue, or you really do not want to discuss it with me first, please contact the LVVP , the professional association to which I am affiliated.
